Applying to different schools on OTCAS can be a daunting task. There are lots of different types of forms that you'll need to fill out, and it can be difficult to know where to start. Here you will find some templates and examples that can help you get started and make your application process go more smoothly.

Observation Hours Log
This is a template that can be used to log your observation hours. To use the template, simply fill out the designated areas. The template is designed to be used for a single observation period. If you are conducting multiple observations, you can create a new template for each observation period.

Community Service Log
This is a template you can use to track your volunteer service for our organization. By keeping this form, you'll be able to keep track of the places you volunteered at, the dates of service, and the number of hours you served. This information will be invaluable when you fill out your OTCAS application.

UT Health Observation Log
This form is required for applications to the University of Texas at San Antonio Health Science Center and must be submitted through OTCAS. Go to the OTCAS website, click "Program Material," then "UT Health San Antonio," then "Documents,". There you will click on "Cover Letter" to add your form.
